I utterly agree with you doc.Just keep the hormones in the physiological levels.Progesterone is a natural antidepressant and also has diuretic and analgesic potential. But should not be used as therapeutic unless the deficiency is clearly documented.Hormones are like a family.They take care of each other.More importantly they follow the demand and supply relationship. Just start demanding them from your body and you will get the supply unless your supply is exhausted like in aging.So the approach is very much dependent on Age/Sex/Race/Relegion/Occupation/Physical Activity needs/Sexual needs/Stress quotient etc. Underlying pathology has to be considered all the time.Many times you have a normal levels of some hormone but the supportive team is not there then better to strengthen the supportive team rather than supplementing that hormone. Thyroxine, Cortisol and Adrenaline play like this.
in my judgement, if you target progestrone in men to the basal level in men then you are withinn physiological levels. Women and men both need progestrone as a basal level to maintain the hormonal balance. It is only at higher levels does it play a role in menstrual cycle and pregnancy. Also progestrone counter balance estrogen excess.
Hey doc. Clinical experience plays important role here.Never use hormones as antagonist of each other but use them just to balance the patient's internal milieu.It's not necessary that you add progesterone to every patient receiving testosterone.If testosterone alone is improving energy then think twice before adding progesterone to men.So the key is, no mthematics or accounts in prescribing Hormones.It's more your clinical judgement.
